A persistent vitelline artery in an adult. Case report and review of literature

Purpose: A case is presented here of a persistent vitelline (omphalomesenteric) artery originating from the superior mesenteric artery. Results: The artery gives off a branch to the intestinum still within the mesentery, runs freely through the abdominal cavity and reaches the umbilical region along the right medial umbilical fold. The lumen of the persistent vessel is open over the entire length, as can be seen in histological sections. Both umbilical arteries are still present and also have a lumen. Discussion: Basic concepts for the normal and misguided embryonic development of these structures are shown. Finally, possible causes for the development of this malformation and its clinical implications are analyzed. Conclusion: This malformation can be seen as an inhibition of the regression of the right vitelline artery. The knowledge of a persistent vitelline artery is important for surgeons, because this malformation can be a cause of intestinal obstruction.
