The complications of disease will increase during pregnancy due to late diagnosis and treatment. Clinical triad of abdominal pain, abdominal distension and absolute constipation can help to diagnose this condition during pregnancy. In this study, a case of sigmoid volvulus during cesarean section is reported. Case presentation: The patient was a 31-year-old woman with gestational age of 36 weeks and 2 days who had referred to Ardabil Alavi hospital with complains of abdominal pain. She had no symptoms of acute abdomen and complete bowel obstruction at the time of referral. The day after visit, due to recording of uterine contractions and the onset of delivery symptoms underwent emergency cesarean section under general anesthesia. During the operation, because of abnormal bowel distension, a general surgical emergency consultation was requested. Sigmoid volvulus was detected and removed by general surgeon. After stabilization of the symptoms, the patient was transferred to the general surgeon's service and the elective surgery was done in the relevant service. Conclusion: Due to dangerous maternal and neonatal complications of volvulus during pregnancy, it is recommended that one of the differential diagnoses of abdominal distention during pregnancy be sigmoid volvulus.
