Summary:A case of eccrine porocarcinoma on the toe is reported. A 67-year-old female presented with a 3-year history of infiltrated plaque on the left middle toe. Physical examination showed a hard infiltrated plaque covered with crusts in reddish brown color, with broken nail, elevated borders and blurry boundaries, on the nail bed of the left middle toe. Histopathological findings included a demarcated tumor connected to the epidermis with growth towards the dermis, tumor necrosis, large pleomorphic cells with vacuoles and large hyperchromatic nuclei, multinucleated giant cells, pathological mitosis and various degree of ductal differentiation. Immunohistochemistry showed positive for CK7,EMA,CK5/6,p16,p40 and p63, and 60%~90% of Ki-67 positive. The diagnosis was eccrine porocarcinoma. Lesion was removed by Mohs microsurgery, and the follow-up is ongoing.
