| Summary: | Branchiogenic carcinoma arising in a preexisting branchial cleft cyst, which is considered an extremely rare entity and there still remain some controversies regarding the differentiation of branchial cleft cancers. We presented the case of an 80-year-old male with a well-defined, immovable, painless, slowly growing mass on the right lateral of the neck which was successfully removed by the surgical resection. The pathological diagnosis was squamous cell carcinoma originating from the branchial cleft cyst, which was characterized microscopically by a branchial cleft cyst with a benign and malignant transformation.
