We presented the case of an 80-year-old male with a well-defined, immovable, painless, slowly growing mass on the right lateral of the neck which was successfully removed by the surgical resection. The pathological diagnosis was squamous cell carcinoma originating from the branchial cleft cyst, which was characterized microscopically by a branchial cleft cyst with a benign and malignant transformation.
