| Summary: | Abstract This is a high-fidelity simulation case with video stimuli of an actual patient (used with permission). The learner evaluates the patient with pain and swelling in the mouth and neck and needs to recognize a deep space infection, which in this case is Ludwig's Angina. Upon recognition, the learner needs to consult an oral maxillofacial surgeon; initiate intravenous antibiotics and steroids; prepare for potential airway deterioration; and admit the patient to the intensive care unit. To the authors' knowledge, this is the first published case with a supplemental video of the actual patient on presentation. On a qualitative level, we have found that providing a video clip of the actual patient increases the fidelity of the case for the learner. Instead of relying on the mannequin and the voice of the patient, learners can immediately see the problem with the patient's intraoral swelling through the video and hear the impressive voice change. This impresses upon them the significance of the physical findings. It is important to mimic the voice of the patient as you hear it on the clip. The most common differential diagnosis considered by the learners who have experienced this case is allergic reaction. |
