Summary:Disease classification system is a grouping of diseases that are similar to the International Statistical Classification of Diseases and Related Health Problems Tenth Revisions (ICD-10) for the term of diseases and health-related issues. The coding application should be appropriate with ICD-10 in order to obtain the codes accuracy because the results are used for indexing the recording of disease, national and international reporting of morbidity and mortality, the analysis of health care financial, as well as for research and clinical epidemiology. The purpose of this study were to know the correlation between coders (doctors and nurses) and the accuracy of diagnosis codes based on ICD-10 at Puskesmas Gondokusuman II Yogyakarta in 2012. The type of this study was the observational analytic study with cross sectional design. The population was the entire files of outpatient medical records for the period of January to June 2012. The number of sample in this study was 385 samples using simple random sampling technique. The analysis showed that the number of codes which accurate were 174 (45.2%) and the inaccurate codes were 211 (54.8%). The results of chi square test with continuity correction showed that Ho was rejected because of the value of p = 0.001 (p <0.05). This showed the correlation between coders (doctors and nurses) and the accuracy of the diagnosis codes.
