Puskesmas is one of the functional organization which organizes the efforts of health care that is comprehensive, acceptable and affordable by the community. Basically Puskesmas Sambungmacan 1 Sragen does not yet have an adequate information system, officers still difficulty in inputing data and convey information about the health of the community. Look at the problems the author will try to create an information system that will facilitate the course activities will support the Ministry in such puskesmas. It contains information in the system tables of the results of the input from the officer who stored and processed with the MySQL database and PHP so that produce useful information, there are also monthly reports and SMS Gateway facility to convey information. This information system is expected to help officials to make it easier inputing data and convey information to the public and improve services in Puskesmas Sambungmacan 1 Sragen. Previous research has still not there is sms gateway to convey information more quickly to the community [7,8]. The results of this research-based information system is an SMS Gateway, PHP and MySQL in the form of a web application. Information system has been through a testing phase through the black-box method and questionnaire.
