Summary:Type 2 diabetes mellitus is caused the insulin target cells are not able to respond to insulin normally. One of the actions taken for the treatment of this disease with pharmacologic therapy. If not properly, this can aggravate or cause new problems for the patient. Potential drug related problems is a conditions where drug therapy is theoretically have the potential to disrupt the goal therapy. This study aims to look at the potential drug related problems include drug interaction and unappropriate drug selection include effective drug but unsafe, ineffective drugs and combinations of drugs that are not appropriate in patients with type 2 diabetes mellitus patient at "X" hospital in 2015. This research is an observational study to collect medical records retrospectively in hospitalized patients with type 2 diabetes mellitus at "X" hospital, then analyzed with descriptive methods. Sampling was conducted using purposive sampling and obtained a sample of 48 patients. The results of analysis of 48 patients were found potential drug interaction were 39 patients (81.25%) with 117 cases and unappropriate drug selection were 33 patients (68.75%) with 86 cases. The unappropriate drug selection because the drug is effective but not safe. Potential drug interactions were the most common is the interaction between metformin with insulin aspart as much as 18 cases.
