520 |a Supply chain system is one of the most important processes for a company. The better the process, the better the level of company efficiency and effectiveness. Supply chain system requires its divisions to always communicate with each other in sending relevant information. The many divisions with various activities in their path make the supply chain process complicated. This supply chain problem is one of the work constraints experienced by PT X. Having an integrated supply chain information system is considered very helpful for process of communication between divisions and reduces the risk of errors. Information stored in the information system can also be used as a reference for users to make a decision. This study aims to design a supply chain management information system at PT X which can be used to assist decision making. Data collection techniques using interviews and questionnaires distributed to every division of the supply chain. The design of management information systems is done using the System Development Life Cycle model. System Development Life Cycle describes in detail the characteristics purpose of management information system design and how to apply it. The management information system design that is generated from the System Development Life Cycle model is presented with Use Case diagram as one of the Unified Modeling Language diagrams and Data Flow Diagram. This study also provides recommendations for decision making relating to the supply chain process at PT X.
