Intravenous antibiotics work briefly, but if converted to peroral, the cost of antibiotics and length of hospitalization can be reduced. Cost effectiveness calculations can determine effective treatment in terms of benefits and costs. This study aims to determine the cost-effectiveness of intravenous levofloxacin converted to peroral and non-converted intravenous levofloxacin in typhoid fever patients. The design of this study is descriptive-analytical with purposive and retrospective sampling using medical records from January 2020 - November 2022 for typhoid fever inpatients at Trimitra Cibinong Hospital, Bogor. The Mann-whitney test compared the length of hospitalization and antibiotic costs. Cost-effectiveness was calculated based on ACER (Average Cost Effectiveness Ratio). There were 15 patients in the conversion group and 21 patients in the non-conversion group, the length of hospitalization was faster in the conversion group (3.40 vs. 3.62), still the difference was not significant (p-value = 0.284). The conversion group had a lower antibiotic cost (IDR 130,879.27 vs. IDR 350,413.52) and was significantly different (p-value = 0.000). Cost-effectiveness was cheaper in the conversion group (Rp38,493.90/day vs Rp96,779.31/day). This concluded that conversion of intravenous levofloxacin is more cost-effective than without converting.
