An ischemic stroke is caused by a blood clot blocking a blood vessel in the brain. This is the most common type, about 80% of strokes are ischemic strokes. Research Methods: This research uses a case study method. Data collection on Mr.D's family was carried out by interview, observation and physical examination. The results of the study: Ineffective Family Health Management is related to the inability of family members to understand the health problems suffered by Mr. D. Physical Mobility Obstacles are related to the inability of the family to care for sick family members, especially Mr. D. Conclusion: Nursing actions until evaluation in Mr. D's family achieved the expected results. Mr. D's family achieved the expected results.
