Summary:Tower cranes are commonly used in construction projects. Tower crane accidents have significant impacts not only on workers at the construction site but also on people outside the construction area. This construction company has experienced a fatal accident related to a tower crane, resulting in the death of a worker who was struck by a concrete bucket that detached from the crane's hook. The incident was indicated to be caused by human error. The objective of this research is to assess human reliability in tower crane operations at Project Construction PT X using Human Error Assessment and Reduction Technique (HEART) method. This type of research is qualitative research. Based on the conducted research, the task with the lowest human reliability score was when the rigger provided instructions during the operation, scoring 0.37, indicating a possibility of inadequate communication. Recommendations to enhance worker reliability include ensuring that informants have sufficient rest and consistently use Personal Protective Equipment (PPE). Additionally, PT X should recruit specialized riggers who hold Occupational Health and Safety (OHS) licenses and competency certificates in their field, improve the shifting system for crane operators, install cooling systems in crane cabins, and conduct stricter supervision on every work activity.
