Background: The third trimester of pregnancy starts from 27-40 weeks (7-9 months). Entering the third trimester, mothers experience discomfort, one of which is back pain. Back pain is often felt in the lumbosacral area due to the growth of the fetus in the mother's uterus. This research was conducted to find out the causal factors and how to overcome back pain with nursing care to be able to reduce the incidence of back pain in third-time pregnant women. Methods: this study used a simple descriptive form of a case study implementing nursing care for Mrs. T (G1P0A0) with back pain problems in the third trimester of pregnancy in the Kramat Jati environment, East Jakarta on 22 February 2023-24 February 2023. Data collection was carried out by interviews, observation and physical examination and documentation. Result: The main nursing diagnosis is acute pain related to physiological changes, the measurement of pain scale is in the form of a numeric rating scale/NRS. The main action taken is in the form of pregnancy exercise to overcome the problem of back pain. Conclusion: This nursing care uses a pain scale measurement with a numeric rating scale/NRS and its documentation with SOAP. Purpose: to gather the client's experience and knowledge about potential pregnancy problems, especially those that arise in the third trimester, and how actions are needed to reduce or overcome them. Result: nursing diagnosis is acute pain associated with physiological changes, pain scale measurement with a numeric rating scale/NRS. The main nursing action taken is in the form of pregnancy exercise to overcome the problem of back pain. Conclusion: This nursing care uses a pain scale measurement with a numeric rating scale/NRS and SOAP documentation.
